Checklist – Guide to filing a petition
Print this checklist to use as a guide to the steps necessary to have a Confidential Intermediary (CI) appointed by the court.

File with the Illinois Adoption Registry
Illinois law requires that you register with the Illinois Adoption Registry and Medical Information Exchange before submitting a petition requesting the appointment of a Confidential Intermediary.
Note: If you are a non-surrendered birth sibling searching for a brother or sister who was adopted, and the birth parent you share is still living, you are not eligible to register with the Illinois Adoption Registry and Medical Information Exchange. However, you are still able to use this program.
How to submit the petition to the Court (for Courts throughout Illinois)
Most people using the CI program have successfully submitted a petition to the Court without the help of an attorney. This section provides step-by-step instructions for completing forms to petition the Court. While CISI cannot provide legal advice, program staff can answer questions about the forms and help you through the process.
If you prefer to file with the assistance of an attorney, your local Bar Association may be able to refer you to an attorney familiar with the Confidential Intermediary program.

The forms you need
The forms for filing can be filled in on-line, then printed out to sign, notarize and submit to the Court. Or you may print the forms from this site and fill them in, or request printed forms from CISI.
